Well, Professor, this is not quite an answer, and more of a way of having a sample answer were it to exist.

I am not sure a gesture would do, unless it could be triggered to stay raised and lowered. And it requires a teacher to continually scan visually among avatars to look for hand raises. I might suggest using something a bit less animated, what people use in online chats, such as some sort of chat message students could type to indicate they have a question; often this ia an exclamation point.

It might be possible to script some sort of object that students could message to indicate a hand raise, and maybe a colored bubble or floating text could be there to attract your attention. We've heard of some places where therre is some sort of talking stick that is used so participants can IM it to indicate they need to speak.

It needs to be simple for students to trigger, and not to disruptive for the teacher to get a nudge, without a teacher needing to divide attention even more.
